Reproduktive Gesundheit in der Weltgesellschaft zwischen Politisierung und Rationalisierung

Zusammenfassung

Im Anschluss an ein neoinstitutionalistisches Verständnis von Globalisierung wird in dem Beitrag reproduktive Gesundheit als makrokulturelles Konstrukt innerhalb eines globalen institutionellen Feldes begriffen, in dem Vorstellungen zum angemessenen Umgang mit Sexualität, rechtliche Rahmenwerke zur Implementierung von reproduktiver Gesundheit in nationalen Gesundheitssystemen und Auffassungen über die Funktionsweise von Sexualität oder Geburt global verankert werden. Zunächst wird gezeigt, dass für die globale Ausdifferenzierung eines institutionellen Feldes der reproduktiven Gesundheit ein menschen- und frauenrechtliches Verständnis von reproduktiver Gesundheit zentral war, das in den 1990er Jahren von institutionellen Unternehmern global verbreitet wurde. Allerdings ist in der Folge ein diskursiver Wandel zu beobachten, der dazu führt, dass ein medizinisch-wissenschaftliches und zunehmend formalisiertes Verständnis von reproduktiver Gesundheit die Oberhand gewinnt. Der Beitrag zeichnet diesen diskursiven Wandel am Beispiel der Redefinition von reproduktiver Gesundheit in den Millennium Development Goals (MDG) nach und interpretiert ihn als Folge einer diskursiven Rationalisierung des institutionellen Feldes. Während der Neoinstitutionalismus in der Regel kulturelle Rationalisierung als Diffusion rationaler Deutungsmuster begreift, wird in diesem Artikel gezeigt, dass sich innerhalb eines spezifischen institutionellen Feldes bestimmte Rationalitätsformen aufgrund ihrer feldspezifischen Autorität durchsetzen.

Das ergebnisorientierte Management ist seit den 1980er Jahren wichtig in der Reform öffentlicher Verwaltungen geworden. Zentral ist die „objektive“ Messung der Erreichung von Zielen. Dabei werden fortwährend neue Ziele und Indikatoren identifiziert und überwacht. Ergebnisorientiertes Management wurde in starkem Maße auf die MDGs angewandt (Hulme 2007).

Zu den gängigen Prinzipien der evidence-based advocacy siehe Eron Friedlaender und Flaura Winston (2004). Heute bieten zahlreiche Lobby-Organisationen ihren lokalen Partnern in den Ländern des globalen Südens Weiterbildungen in evidence-based advocacy an, damit diese ihre Verhandlungsposition gegenüber Regierungsstellen verbessern können (Storeng und Béhague 2014, S. 270).

Das Millennium Project war an der Columbia-Universität angesiedelt und erstellte eine Studie zur Implementierung und Finanzierung der MDGs. Mit Unterstützung von UNDP und UNFPA erstellten Experten zehn Task-Force-Berichte zu verschiedenen Themen, unter anderem auch zur Mütter- und Kindergesundheit.
