Zusammenfassung

Schwindel wird im Alter häufig angegeben und ist daher ein für die Neurogeriatrie relevantes Syndrom. Mehr als 80 % der über 80-jährigen Personen haben Gleichgewichtsstörungen. Hinter der anamnestischen Angabe von Schwindel verbergen sich spezifische peripher- und zentral-vestibuläre Erkrankungen, aber auch andere Ursachen wie Blutdruckregulationsstörungen, Medikamentennebenwirkungen, multisensorische Syndrome oder Angst-assoziierte Gleichgewichtsstörungen. Besonders häufig und therapierelevant ist der gutartige Lagerungsschwindel, der immer ausgeschlossen werden sollte. Die Behandlung erfolgt mit spezifischen physikalischen Manövern. Häufig sind auch sensorische Defizite (Polyneuropathie, Vestibulopathie), beginnende neurodegenerative Erkrankungen (Parkinson, Kleinhirnataxie) und der zentrale Schwindel bei vaskulärer Enzephalopathie. Die exakte Diagnose der Teilursachen eines Schwindelsyndroms erlaubt die gezielte Therapie, die häufig Gleichgewichtstraining einschließt und nur selten eine medikamentöse Behandlung erfordert.
