Zusammenfassung

Ein unfreiwilliger Urinverlust (Inkontinenz) gehört zu den häufigsten und besonders belastenden Folgen einer neurologischen Erkrankung im Alter. Die Inkontinenz führt zu sozialem Rückzug, Folgeerkrankungen, depressivem Verhalten und kann den Anlass für eine Heimunterbringung darstellen. Häufig stehen zunächst andere Funktionsdefizite im Vordergrund – die Basisdiagnostik einer neurogenen Harninkontinenz sollte jedoch in jeweils neurologische, geriatrische oder urologische Behandlungen eingebettet sein. Die Therapie umfasst neben verhaltenstherapeutischen auch medikamentöse und operative Interventionen und die Kombination von allen genannten. Das Behandlungskonzept sollte nach den Empfehlung der ICI ‚theory based‘, individuell, multi-komponent, multidisziplinär und Patienten-zentriert sein, also berufsgruppenübergreifend und interdisziplinär festgelegt werden und den neurologische Befund, die Ressourcen und Wünsche des Patienten sowie seiner Betreuungspersonen einbeziehen.
