Abstract

The article addresses the intricacies of individually randomized group-treatment (IRGT) trials, where patients are randomized individually but treated in groups, leading to correlated outcome data within the experimental arm. It builds upon existing methods for cluster randomization trials (CRTs) and subunit randomization trials (SRTs), extending them to accommodate the distinct dependency structures of IRGTs. The proposed modified log-rank test and its sample size calculation method are designed to handle time-to-event outcomes, accounting for various factors such as marginal survival distributions, bivariate survival and censoring distributions, and cluster size distributions. The article also explores optimal allocation proportions to minimize total sample size or cost, providing practical insights for trial design. Through extensive simulations and a real-world example, the article demonstrates the robustness and accuracy of the proposed methods, making it an essential read for those involved in the design and analysis of complex clinical trials.
