Electronic health record (EHR) systems have acquired an outstanding role on the evolution of healthcare. The wide spectrum of efforts (of implementation and standardization) is a sample of such importance. EHR systems by itself are often considered as the ultimate goal of IT policies in health, by disregarding the potential of acting as supporting building blocks of advanced systems (e.g., knowledge generators). Thus, to promote the cooperation and interoperability among EHR systems and others is a major challenge on the road ahead of health IT systems. The use of service architectures could ease the interoperability, but the integration of EHR systems (even standardized ones) within an ecosystem or service architecture is a not deeply explored issue. This work presents a formalization approach for one of the most relevant international initiatives in EHR systems, the family of standards EN/ISO 13606. This formalization is made according to the methodological principles of the ISO 10746 standard (RM-ODP) and integrated in a health service architecture conformed to the EN/ISO 12967 standard (HISA).
