Zusammenfassung
Im vorliegendem Beitrag werden die klinisch relevanten Entwicklungen im Bereich der Zerebralparesen (CP), der häufigsten motorischen Behinderung zerebraler Ursache im Kindesalter, dargestellt. Diese betreffen v. a. die Definition und Klassifikation sowie die diagnostische Rolle der zerebralen Bildgebung. Eine Arbeitsgruppe in Europa (SCPE) empfiehlt eine einfache Definition und Klassifikation, ausgerichtet an Schlüsselelementen gegenwärtiger CP-Definitionen. Die Einteilung erfolgt nach neurologisch definierten Subtypen, spastisch, dyskinetisch und ataktisch, und erfasst den Schweregrad anhand von standardisierten, funktionellen Scores. Leitlinien zur Diagnose der CP empfehlen die zerebrale Bildgebung, insbesondere die MRT, als erste diagnostische Maßnahme (nach Anamnese, neurologischer Untersuchung und Feststellen von eventuellen Zusatzproblemen). Bei etwa 85% der CP ist ein Befund zu erwarten, der die Pathogenese oder Ätiologie der Erkrankung erklärt, wobei erworbene Ursachen bei Weitem vorherrschen, genetische selten sind. Dies hat konkrete Konsequenzen für die Familien bezüglich genetischer Beratung einerseits und zur besseren Prognoseeinschätzung andererseits.
Abstract
This article provides an update on clinically relevant developments in the domain of the cerebral palsies (CP), the most frequent motor disabilities of cerebral origin in childhood. It considers definition and classification, as well as the diagnostic role of neuroimaging. In Europe, a working group (Surveillance of Cerebral Palsy in Europe, SCPE) has suggested a simple definition and classification based on key elements of current CP definitions. The classification involves neurologically defined subtypes, spastic, dyskinetic and ataxic, and defines severity on the basis of standardized functional scores. Guidelines for the diagnosis of CP suggest neuroimaging, especially magnetic resonance imaging, as the first diagnostic procedure after history taking, neurological investigation and investigation for other potential disabilities. In around 85% of CP cases, a finding explaining the pathogenesis or etiology of the disorder is to be expected, acquired origins being predominant and genetic origins rare. This is relevant for genetic as well as prognostic counseling.
