This book adopts a case-based or story-based approach to addressing questions such as what happens when we cannot fix failures in the sense of returning the system to “normal” operations in less than many years or maybe decades, or perhaps never, or how do we recognize that we are facing pervasive failures? Can we develop evidence-based resilience to mitigate the consequences of pervasive failures? The five domains explored herein include banking and finance, environment, health and well-being, infrastructure, and organizations. Each of chapters begins with a lead story that in one way or another, involved the author. Beyond these lead stories, the book discusses over 40 other cases in these domains. Dr. Rouse articulates an analytic framework for characterizing stories including ten central questions, as well as a multi-level modeling construct extended to address wicked problems and complex adaptive systems. This provides the foundation for outlining, in the last chapter, an integrated approach to decision support to enable evidence-based resilience for all the cases discussed.

The government bailed out the culprits, but not the victims. -

William B. RouseAbstractGrowing up in a small seacoast town in Rhode Island, I experienced several hurricanes, especially Carol (1954), but also Cindy (1959) and Donna (1960). Superstorm Sandy (2012) provided an amazing introduction to Hoboken, having just moved there. The recent devastation of Hurricane Helene (2024) in Western North Carolina demonstrated the ultimate power of water—it always wins. -
5. Health and Well-Being
William B. RouseAbstractData from CDC’s National Center for Health Statistics indicate there were an estimated 108,000 drug overdose deaths in the United States during 2023. Opioid overdose deaths were 81,000, a slight decrease with fentanyl making up the difference. How did this pervasive failure happen? -
6. Infrastructure
William B. RouseAbstractThe terrorist attacks of September 11, 2001 resulted in enormous physical destruction of buildings and infrastructure in New York City and Washington, DC. These consequences called into question the organizational infrastructure of the US for detection, prevention, protection, interdiction, containment, attribution, analysis, and intervention capabilities to address such threats. Failures to organize, integrate, and operate such capabilities were pervasive and remain an ongoing challenge, particularly because of the fragmented organizational nature of our country. -
7. Organizations

RouseAbstractExpectations for aviation safety are such that two fatal accidents abroad in 2018 and 2019 involving U.S.-manufactured aircraft caused Congress to ask the National Academies of Sciences, Engineering, and Medicine to review existing data and processes for identifying anomalies before they can potentially turn into future accidents. -
8. Decision Support
William B. RouseAbstractIt is important that all stakeholders perceive that pervasive failures are recognized, understood, and are being appropriately addressed. Such perceptions need to be fostered despite the chaos of misinformation and disinformation being generated by social media, and exacerbated by political polarization. I return to this challenge in this chapter. -
