28. Blutreinigungssysteme

Zusammenfassung

Der Anstieg der Zahl von chronisch nierenkranken Patienten hat die Entwicklung der Hämodialyse zu einer Routinetherapie gefördert. Entscheidende Verbesserungen des Dialysesystems, von der Membran über das Schlauchsystem bis hin zum Dialysemonitor, haben die Grundlage dazu gelegt. Das Verständnis der zugrunde liegenden Stellgrößen und deren Variation im Sinne einer optimalen Therapie hat auch dazu geführt, dass die früher als normal geltenden Nebenwirkungen des Dialyseverfahrens reduziert und damit die Lebensqualität verbessert und die Mortalität verringert werden konnten. Dazu gehören innovative Verfahren, die einzelne physiologische Parameter mithilfe von Feedbacksystemen analysieren und über die Dialysemaschine steuern. Diese sind heute Stand der Technik. Auch für Kranke mit Leberversagen werden heute extrakorporale Blutreinigungsverfahren eingesetzt. Im Gegensatz zur Hämodialyse verwendet man hier proteinpermeable Membranen mit denen albumingebundene Toxine filtriert werden, deren Aufreinigung im Filtratkreislauf durch Adsorberkolonnen gelingt. Klinische Untersuchungen haben die Wirksamkeit dieses Verfahrens bestätigt.
