Perhaps it was chance, but I completed reading this intriguing book the day before Memorial Day Weekend, 2017. As I was watching the ceremonies commemorating the fallen American military personnel, I could not help but wonder how many of those American dead had been the victims of unintended actions by their military comrades. I recalled the media accounts of the death of Pat Tilman, a professional football player who enlisted in the military following the terrorist attack on the Twin Towers in New York City on September 11, 2001. In 2004 he was killed by friendly fire. The event not only received wide media coverage, but also was the subject of a Congressional investigation. …
