Polar-Based Aortic Segmentation in 3D CTA Dissection Data Using a Piecewise Constant Curvature Model

Immediate open surgery represents nowadays the only possibility to treat acute aortic Type A dissections, involving the ascending aorta. However, this procedure is correlated with high perioperative mortality rates. Due to the complex vascular anatomy of the ascending aorta and aortic arch, a minimally invasive therapy would only prove feasible by manufacturing custom-designed stent-grafts for each patient according to morphological characteristics obtained from CT data. In order to overcome the inherent difficulties linked to the segmentation of severe aortic dissections, this contribution introduces a novel polar-based segmentation approach implying a piecewise constant curvature model for the outer aortic cross-sectional boundary. Subsequently, the resulting aortic mesh is refined by an efficient narrow-band 3D level-set method.
