Firefighting is strenuous work that causes significant strain on the heart and cardiovascular system. As a result, firefighters are disproportionately affected by serious cardiac events when compared to other emergency personnel [1, 2]. Nearly 50% of all duty-related fatalities among firefighters since the 1970s are due to sudden cardiac death (SCD), a fatal tachy-arrhythmia that leads to the cessation of blood flow in the body [35]. Although fire suppression activities comprise approximately 5% of total annual working time in firefighters, SCD most likely occurs during or immediately after these activities [5, 6]. Exacerbating the elevated risk of SCD is the high prevalence of obesity, hypertension, and other chronic conditions among firefighters [8]. Thus, firefighting in combination with underlying chronic conditions strains the heart and precipitates serious cardiac events, including SCD (Fig. 1).
